Survey

You are invited to participate in a research study entitled Organizations in Transition: West Side Christian Church. This study is being conducted by Rachel Widener at the UNC Charlotte Communication Studies Graduate Department under the supervision of Dr. Loril Gossett.  Eligible participants include West Side Christian Church congregational members over the age of 18.

Please read the information below and ask any questions you might have before deciding whether or not to take part. Your participation is entirely voluntary. You can refuse to participate without penalty or loss of benefits to which you are otherwise entitled. You can stop your participation at any time and your refusal will not impact current or future relationships.

The purpose of this study: To address the organizational challenges encountered by WSCC.

Estimated time to participate: The survey should take approximately 15 minutes to complete.

Compensation: There is no compensation offered for participation in this study.

Benefits and Risks: You are a volunteer. The decision to participate in this study is completely up to you. If you decide to be in the study, you may stop at any time. You will not be treated any differently if you decide not to participate or if you stop once you have started. Although there are no reasonably foreseeable risks, this project may involve risks that are not currently known. Reasonably foreseeable benefits include improved organizational communication, understanding, and successful transition.

Confidentiality and Privacy Protections: All information obtained in this study will be kept  confidential. In no case will any individual names be reported. If you would like to see the results of the study, you may contact the primary investigator (listed above). The data resulting from your participation may be made available to other researchers in the future for research purposes not detailed within this consent form. In these cases, the data will contain no identifying information that could associate you with it, or with your participation in any study.
